Referrals
What To Expect
Once CenterCare Health and Wellness has received a referral, our staff will verify the person’s insurance information, and then contact the person regarding their financial responsibility for services. If they accept, they will be scheduled with a licensed clinician to determine their eligibility for services at CenterCare. If determined eligible for services, the person will then be scheduled for counseling with a licensed clinician and/or for a Psychiatric Evaluation and if applicable, medication management follow ups with a Psychiatrist or Advanced Nurse Practitioner. If the person is not eligible for services at CenterCare, recommendations or a referral may be made to another community agency.
CenterCare Health and Wellness accepts the following insurance and paysources:
Confirmation of In-Network status is advised as we are continually being added as a provider to group health plans.
