CenterCare Health and Wellness is now accepting referrals for the following array of services:
- Psychiatric evaluation and treatment
- Medication management
- Individual Therapy
- Laboratory Services
- Medication Administration
To submit referrals, email: centercare@chcsbc.org, or fax referral to 210-261-1837.

What To Expect
Once CenterCare has received a referral, financial/insurance information will be verified, and an initial assessment will be made to determine eligibility for services. If CenterCare is not the right match, recommendations or a referral may be made to another community agency. Some exclusionary diagnosis may include but is not limited to Schizophrenia, IDD, Asperger Syndrome and Autism.
